Fill Forms
Page Overview
What the page is for
The Fill Forms page lists the forms (surveys) that have been shared with you and lets you start a new one. It is the entry point for field work: browse the available forms, then click Start Form to begin filling it out.
From here you can also open the Resume Center to continue drafts, paused sessions, and unfinished interviews, and reach a form's Manage Access and View Responses pages (administrators only).
When a user would use it
- When you want to fill out a new form
- When you need to find a specific form among many you can access
- When you want to jump to the Resume Center to continue work you started earlier
- When you (as an administrator) need to manage who can fill a form or view its responses

Step-by-Step Tasks
Starting a new form
- Find the form you want to fill out in the Available Forms list.
- Click Start Form.
- If the form supports pre-filling, a prompt appears asking how to start:
- Start fresh: begin with an empty form.
- Pre-fill with your respondent ID: pre-fills fields linked to your respondent ID.
- Pre-fill with a unique ID: pre-fills fields linked to a specific unique identifier.
- The form opens and you can begin filling it out.
Previewing a form

- Click Preview.
- The form opens in preview mode (the URL includes
?preview=true). No response is saved.
Continuing work you started earlier
- Click Resume Center in the toolbar.
- Continue a draft, paused session, or unfinished interview from there.
- To pick up an individual form directly, use the Resume or Start button on its card in the Resume Center.
Searching for forms
- Click in the search bar at the top of the page.
- Type part of the form name.
- The Available Forms list filters automatically.
Refreshing the list
- Click Refresh in the toolbar.
- The list reloads with the latest forms.
Managing access (administrators only)
- Find the form you want to manage.
- Click the three-dot menu (⋮) on the card.
- Select Manage Access.
- Grant, edit, or revoke access levels and assignment triggers as needed.
Assigning lookup triggers to enumerators (administrators only)
If a form has lookup data with trigger values (for example, household IDs), you can assign specific trigger values to individual enumerators. This limits each enumerator to only see the lookup rows for the values assigned to them.
Viewing responses (administrators only)
- Find the form you want to check.
- Click the three-dot menu (⋮) on the card.
- Select View Responses.
- You are taken to the form's responses page.
Duplicate Response Warning
When you start a form that supports duplicate checks, MQUAD searches for responses that look similar to the one you are about to create. If a possible duplicate is found, a warning dialog appears before you begin. You can review the matches and decide whether to continue. This helps prevent the same respondent from being recorded twice.

Tips & Notes
About access
- You only see what you've been granted: If a form you need is missing, ask an administrator to grant you access.
- Can Fill vs View Only: The badge on each card tells you whether you can submit responses or only view the form.
About versions
- Always use the version shown: The version badge (for example
v3) tells you which version of the form is current for field work.
About advanced or anonymous forms
- Anonymous forms don't record your identity with the response.
- Multiple responses: Some forms allow more than one submission from the same user - look for the "Multiple responses" badge.
Where other work now lives
- Drafts and sessions: Continue them from the Resume Center (
/resume). - All interviews: See the Interviews page (
/interviews) and the Interview Lifecycle guide. - Submitted responses: See the Data Viewer, and learn how collection works in Collecting Responses.
- Rejected responses: When a response needs revision, reopen it from the interview (the interview shows a Resume Revision action) and refer to Response Editing.
